Do not use names; just male caucasian for example. The only difference is that  they are requesting to add social determinant of health. 

I liked how you did the last one, so something similar. you can create your own scenario such as in the mental status examination and other sections or add something you have room on this one since its supposed to be for a patient who the instructor is not aware of.
